Citizens of Surat who have recent travel history of Abroad / States affected from COVID-19, and has received SMS with Traveller ID and other necessary details from sender ID SMCORP are requested to download this application and send their GPS location every hour (From 9 AM to 9 PM) to SMC and submit the questionnaire provided in application two times every day with Selfie.
Note: In case GPS location is not shared by Home Quarantined Citizen as described above, SMC will be compelled to shift such citizen to SMC Quarantine Center. Non-cooperation will be subject to leavy of Rs. 25,000 as penalty under the Epidemic Diseases Act, 1897.
સુરત શહેરના નાગરીકો જેમણે હાલમાં જ COVID-19થી પ્રભાવિત દેશ અથવા રાજ્યમાં પ્રવાસ કર્યો હોય સુરત મહાનગરપાલિકાના SMS એકાઉન્ટ SMCORP તરફથી Traveller ID સહીતની વિગતો મળી હોય તે નાગરીકોને ઉપરોક્ત મોબાઈલ એપ્લીકેશન ડાઉનલોડ કરી તેઓનું GPS લોકેશન દર કલાકે (સવારે ૯ થી સાંજે ૯ વાગ્યા સુધી) ફરજીયાત મોકલાવવું તથા દિવસમાં ૨ વાર એપ્લીકેશનમાં આપેલ ફોર્મ સેલ્ફી સાથે ભરવું.
નોંધ: હોમ કવોરાનટાઈન નાગરિક દ્વારા ઉપર જણાવ્યા મુજબ નિયમિત GPS લોકેશન મોકલવામાં નહિ આવે તો તેઓને સુરત મહાનગરપાલિકાના Quarantine સેન્ટર ખાતે શિફ્ટ કરવાની ફરજ પડશે. આ બાબતે અસહકાર ધી એપેડેમીક ડીસીઝીસ એક્ટ ૧૮૯૭ અન્વયે રૂ. 25,૦૦૦ ની પેનલ્ટીને પાત્ર થશે.
